A producer seeking to register a pesticide under 18 AAC 90.215 shall submit an application to the department on a form supplied by the department and the fee required by 18 AAC 90.850. The application must include
(1) the applicant's name, telephone number, and address;
(2) the name, telephone number, and address of the person whose name will appear on the label, if not the applicant's;
(3) the name and EPA registration number of the pesticide;
(4) a complete copy of the registration label stamped by the EPA;
(5) a complete copy of the product labeling;
(6) the correct name and total percentage of each active ingredient;
(7) the total percentage of inert ingredients; and
(8) other pertinent information requested by the department.
