(a) A registrant, at the request of any individual formerly employed or associated with him shall furnish to such individual a report of his exposure to ionizing radiation as shown in records maintained according to sec. 330(a) of this chapter. Such report shall be furnished within 30 days from the time the request is made; shall cover each calendar quarter of the individual's employment or association involving exposure to ionizing radiation, or such lesser period as may be requested by the individual. The report shall be in writing and contain the following statement:
"This report is furnished to you under provisions of Alaska Department of Environmental Conservation regulations. You should preserve this report for future reference."
(b) The individual's request should include appropriate identifying data, such as social security number and dates and locations of employment or association.
