Each registrant shall supply the appropriate personnel monitoring equipment to, and shall require the use of such equipment by:
(1) any individual, except a patient, who enters a controlled area under such circumstances that he receives, or is likely to receive, a dose in any calendar quarter in excess of 25 percent of the applicable value specified in Table III in sec. 130 of this chapter;
(2) any individual under 18 years of age who enters a controlled area under such circumstances that he receives, or is likely to receive, a dose in any calendar quarter in excess of five percent of the applicable value specified in Table III of sec. 130 of this chapter; and
(3) any individual who enters a high ionizing radiation area.
