(a) The owner or operator of a Class A or Class B public water system that uses a surface water source or a GWUDISW source shall report in writing to the department within 10 days after the last day of each month during which the public water system serves water to the public, subject to the report certification requirements of 18 AAC 80.1900. Subject to 18 AAC 80.675(d) and 18 AAC 80.680(c) , the following information must be reported:
(1) for each day, the lowest measurement of residual disinfectant concentration in mg/l in water entering the distribution system;
(2) the date and duration of each period when the residual disinfectant concentration in water entering the distribution system fell below 0.2 mg/l and the date the department was notified of the occurrence in accordance with (d) of this section; and
(3) for the month just ended and the previous month the public water system served water to the public, the information on the samples taken in the distribution system as required by 18 AAC 80.635(c) (4).
(b) If a waterborne disease outbreak occurs, or if an outbreak is suspected to have occurred, the owner or operator of a Class A or Class B public water system shall report the occurrence to the department as soon as possible, but no later than by the end of the next working day, subject to the report certification requirements of 18 AAC 80.1900.
(c) If the turbidity in the water exceeds five NTUs, the owner or operator of a Class A or Class B public water system shall report to the department as soon as possible, but no later than the end of the next working day, subject to the report certification requirements of 18 AAC 80.1900.
(d) If the residual disinfectant concentration in the water entering the distribution system falls below 0.2 mg/l, the owner or operator of a Class A or Class B public water system shall notify the department as soon as possible, but no later than by the end of the next working day. The owner or operator shall also notify the department by the end of the next working day whether or not the residual disinfection concentration was restored to at least 0.2 mg/l within four hours.
