(a) The registered service provider shall execute a placed in service report, including the registrant's assigned registration number, and sign it, for each rejected device restored to service, or for each new or newly installed device placed in service. A registered service provider shall inform the division within 24 hours after a device has been restored to or placed in service. Within 10 days after a device has been restored to or placed in service, a registered service provider shall provide the director a copy of the placed in service report. A registered service provider shall give a copy of the placed in service report to the owner or operator of the device at the time the device is placed in service, and retain a copy of the placed in service report for two years after the device is placed in service. A placed in service report must be completed for each device that has been placed out of service by the division and has been serviced and restored to service by a registered service provider and for each new or newly installed device placed in service by a registered service provider.
(b) A registered service provider shall affix a security seal, in a manner that requires the security seal to be broken before an adjustment can be made, to any component affecting the performance of any electrical or mechanical device designed to be sealed with a security seal. The seal must bear the identification number assigned to the registered service provider.
