(a) A state employee must complete an application for a limited process server license on a form provided by the department.
(b) Except for an employee of the department, an applicant for a limited license must include the following items with the application form:
(1) two sets of classifiable fingerprints on a Federal Bureau of Investigation Applicant Card (FD-258);
(2) a money order for $43 payable to the department for fingerprint processing; and
(3) proof of notice to the state Department of Administration, division of risk management, that the employee has applied for a limited process server license.
