Applicants for an original school bus driver permit shall, at the time of application, furnish the following to the examiner in order for the application to be accepted:
(1) a completed health history on a form prescribed by the Department of Public Safety;
(2) two sets of applicant fingerprint cards on a form prescribed by the department;
(3) a form certifying successful completion of a state-approved school bus driver training course; and
(4) a money order in the amount required to conduct a fingerprint search.
