(a) An applicant for a temporary license to practice as a veterinarian shall submit
(1) a completed application for a temporary license on a form provided by the department;
(2) the temporary license fee established under 12 AAC 02.350;
(3) a notarized copy of the applicant's veterinary school diploma or Education Commission for Foreign Veterinary Graduates certificate; and
(4) a statement signed by the supervising veterinarian, on a form provided by the department, indicating that the applicant will be employed under the veterinarian's supervision until the applicant's examination results are reported.
(b) An applicant who has received a temporary license under this section must sit for the next regularly scheduled examination. If an applicant is unable to sit for the next regularly scheduled examination, the board, for good cause shown, may extend a temporary license.
