Claims for reimbursement from the real estate surety fund must
(1) be submitted on a form provided by the commission;
(2) be for a loss suffered in a real estate transaction by the claimant as a result of fraud, misrepresentation, deceit, or the conversion of trust funds by an involved licensee;
(3) include all the information required by AS 08.88.460 (a) and by the claim form itself; and
(4) be executed under penalty of unsworn falsification.
