(a) Before an individual can practice as a graduate physician assistant, the individual shall
(1) apply on the form provided by the department;
(2) pay the collaborative relationship fee established in 12 AAC 02.250;
(3) hold graduate physician assistant license issued under 12 AAC 40.445;
(4) submit documented evidence of an established collaborative relationship under 12 AAC 40.410 and 12 AAC 40.980; and
(5) be approved by a member or designee of the board.
(b) When licensed and authorized to practice, the licensee shall display a nameplate designating that person as a "graduate physician assistant."
