A licensed mechanical administrator shall notify the department in writing within 15 days after a change in mechanical administration employment or company affiliation by submitting the change of employer or company affiliation fee established in 12 AAC 02.245 and a notarized statement that includes the
(1) name of the previous company and the date employment or affiliation ended;
(2) name of the new company and the date employment or affiliation began;
(3) construction contractor license number for each company; and
(4) mechanical administrator's license number.
