The following fees are established for optometrists:
(1) nonrefundable application fee for initial license, $50;
(2) examination fee, $50;
(3) license fee for all or part of the initial biennial license period, $420;
(4) biennial license renewal fee, $420;
(5) pharmaceutical agent license endorsement fee for all or part of the initial biennial endorsement period, $100;
(6) biennial pharmaceutical agent license endorsement renewal fee, $100;
(7) repealed 11/15/96;
(8) repealed 11/15/96.
