(a) The following fees for dental hygienists are established:
(1) nonrefundable application fee for initial license, $85;
(2) board conducted examination, $3,000;
(3) active license fee for all or part of the initial biennial license period, $220;
(4) biennial active license renewal fee, $220;
(5) temporary license or local anesthetic permit, $50;
(6) credential review fee, $100;
(7) local anesthetic permit renewal fee, $50;
(8) biennial inactive license renewal fee, $110.
(b) The following fees for dentists are established:
(1) nonrefundable application fee for
(A) initial license by examination, $300;
(B) branch office registration, specialty license, or permit, $50;
(2) board conducted clinical examination, $5,000;
(3) active license fee for all or part of the initial biennial license period, $590;
(4) specialty license fee for all or part of the initial biennial license period, $100;
(5) biennial active license renewal fee, $590;
(6) biennial specialty license renewal fee, $100;
(7) branch office registration, $25;
(8) parenteral sedation permit fee for all or part of the initial biennial permit period, $100;
(9) biennial parenteral sedation permit renewal fee, $100;
(10) biennial branch office registration renewal fee, $25;
(11) nonrefundable application and review fee for license by credentials, $400;
(12) general anesthetic permit fee for all or part of the initial biennial permit period, $100;
(13) biennial general anesthetic permit renewal fee, $100;
(14) biennial inactive license renewal fee, $295;
(15) fee for reactivating an inactive license, $295.
(c) The following fees are established for submission of dental and dental hygiene continuing education courses for approval under 12 AAC 28.410:
(1) initial continuing education course submittal fee, $25;
(2) continuing education course resubmittal fee, $15.
