(a) Before placing the child into a home, the child placement agency must have a written agreement with the adoptive or guardianship parents. A signed copy of this agreement must be given to the adoptive or guardianship parents.
(b) The agreement must specify the following:
(1) the adoptive or guardianship parents and the agency agree to complete the adoption or guardianship within a specified time;
(2) the adoptive or guardianship parents agree to supervision by the agency during the time before completion of the adoption or guardianship;
(3) the adoptive or guardianship parents must notify the agency before removing the child from this state before the completion of the adoption or guardianship;
(4) either the adoptive or guardianship parents or the agency can return the child to the agency at the discretion of either the adoptive or guardianship parents or the agency before the adoption or guardianship is completed;
(5) what the fee and schedule of payment are.
