Information to be safeguarded by the division includes:
(1) names and addresses of recipients of services from the division;
(2) information contained in applications, reports of investigations, evaluations, medical examinations, correspondence and court reports, and other information concerning the condition or circumstances of any person about whom information is obtained, regardless of whether this information is recorded; and
(3) division evaluations of and action taken on the information.
