(a) Each proposal for a residential child care facility grant must include
(1) proof of voluntary licensure under 7 AAC 50.071 for any facility in Alaska at which the proponent proposes to provide services, if the facility is legally exempt from state licensing requirements and is not licensed by the state;
(2) a service plan; and
(3) a proposed budget, including proposed monthly rates as described in 7 AAC 53.965(c) .
(b) The proponent, in its service plan, shall project its activities for the period of the grant, and shall offer to provide services for children for whom the state has assumed responsibility under AS 47.10.
(1) A statement of projected activities must describe
(A) all facilities and programs that the proponent would operate, and the program objectives of the proponent for each facility;
(B) all types of services that the proponent would provide; and
(C) the number of beds that the proponent would provide for each type of service at each facility.
(2) An offer to provide services must designate
(A) the geographic area or community where the proponent would provide services under the grant;
(B) the period for which the proponent would provide services under the grant;
(C) the location of the facility or facilities at which the proponent would provide services under the grant;
(D) the number of beds that the proponent would provide under the grant for each type of service at each facility;
(E) the maximum dollar amount that the department would reimburse the proponent, under the grant, for each bed dedicated to each type of service; and
(F) the manner in which the proponent would meet any special requirements that the department states in the request for proposals and grant application instructions.
(c) In a proposed budget, prepared under 7 AAC 53.995(b), a proponent shall make an estimate of its expenses and receipts for the period of the grant. A proponent shall prepare a proposed budget on forms provided by the department and shall follow the definitions and principles prescribed for reporting in 7 AAC 53.901 - 7 AAC 53.999.
