For services furnished on or after July 1, 2004, the department will not make a payment under the CAMA program for any expense that is
(1) for major medical care;
(2) for nursing facility care;
(3) for items and services not properly prescribed or determined necessary by a health care provider enrolled with the department;
(4) for a person in the care and custody of a federal, state, or local correctional facility;
(5) for an elective procedure;
(6) for services furnished at no charge to an Indian Health Service beneficiary by an Indian Health Service facility or a facility funded by the Indian Health Service;
(7) not directly related to the treatment of a medical condition listed in 7 AAC 48.525(b) ;
(8) for prescriptions after the first three are filled within a calendar month;
(9) for transportation; or
(10) for physician services if the physician services are furnished in an inpatient hospital or in a nursing facility.
