(a) The division will review each application for completeness, including documentation of medical expenses, gross income, and assets.
(b) If an applicant has previously received catastrophic illness assistance, the applicant must document timely payment of the applicant's share for the previous period of eligibility. An applicant who has not paid the applicant's share for a previous catastrophic illness or who is not following a payment schedule is not eligible for additional assistance.
(c) The division will, in its discretion, request more complete information and contact other sources to verify information contained in an application. By applying for catastrophic illness assistance, an applicant consents to release of confidential and other information to the division.
