(a) A person applying for financial assistance under this chapter must apply on a form approved by the committee. The committee may deny an application for failure to complete the application form or otherwise to provide information required by the committee.
(b) Application forms, along with the formula used to calculate the applicant's share, described in 7 AAC 48.070, are available at local offices of the division of public assistance, at hospitals in the state, and at the offices of the division of medical assistance.
(c) Repealed 8/17/84.
