(a) An individual may apply for assistance under 7 AAC 47.800 - 7AAC 47.890 by
(1) applying on a form provided by the department; the department may provide an abbreviated form to an individual who is eligible under 7 AAC 47.805(b) ; and
(2) certifying on a form provided by the department to the accuracy and truthfulness of information provided under (1) of this subsection.
(b) Application may be made by the individual seeking assistance or by a responsible person acting on the individual's behalf.
(c) If an individual wishes to continue to receive assistance each month under 7 AAC 47.800 - 7 AAC 47.890, the individual is not required to submit a new application under (a) of this section.
(d) If the individual discontinues assistance under 7 AAC 47.800 - 7 AAC 47.890, the individual may reapply by submitting the forms specified in (a) of this section.
