The division shall provide a written certification of eligibility for General Relief Medical benefits upon a form or card specified by it. This certification signifies that the person or persons named on it have been found eligible for medical assistance for the period of time stated. This certification neither guarantees to the recipient that certain medical services and supplies will be furnished nor guarantees to the provider that he will receive unrestricted payment for any and all services and supplies he provides.
