The division shall render an eligibility decision upon each identifiable application for assistance and shall forward to the applicant a written notice-of-eligibility decision no later than 30 days from the receipt of the application by a district office. In this section,"identifiable" application means one which contains at least the applicant's name, mailing address, and signature or witnessed mark. However, in order for assistance to be granted, the applicant must complete all portions of the application.
