(a) The grant of General Relief assistance and General Relief Medical assistance may be considered only upon application. Any person, regardless of race, religion, age, sex, or national origin, may apply for this assistance. Applications must be made on a form designated by the division of public assistance. The form is available at any district office of the division and must be filed at a district office. Application may be made either by the individual requesting the assistance or by a responsible person acting on behalf of the individual.
(b) Notwithstanding (a) of this section, the division will not accept an application for General Relief Medical assistance if the application is received in a district office after July 31, 1998.
