7 AAC 45.150. Identifiable application

To receive Alaska Temporary Assistance Program (ATAP) benefits under AS 47.27, an applicant must apply for benefits, in writing or by electronic means, on a form provided by the department. The application form must clearly identify the applicant by including a legible name, mailing address, and signature or witnessed mark, and, as required by AS 47.27.020 (b), must include the attestation regarding other state assistance. The application form must be filed with a district office of the division handling public assistance matters in the department, or a fee agent or other division designee.
