An applicant shall, upon request, provide the division with verification of income and any other information contained in the application. An applicant shall provide verification acceptable to the division through documentary evidence (such as a wage stub, profit and loss statement, or award letter) or, if documentary evidence is not available, through correspondence from disinterested and unrelated third parties such as village council members, social service agency staff, or fee agents.
