(a) The division will enroll as a Medicaid provider of in-state freestanding or portable x-ray services a provider who
(1) is certified by the division under 42 C.F.R. 488.26 as in compliance with 42 C.F.R. 486.100 - 486.110; or
(2) holds a valid mammography equipment certificate issued by the federal Food and Drug Administration under 21 C.F.R. 900.11 (April 1, 1995).
(b) Reimbursement for in-state x-ray services will be made in accordance with 7 AAC 43.040.
(c) Repealed 12/2/2005.
