(a) The department may deny enrollment or certification to, or disenroll or decertify, a person care agency as a provider for the consumer-directed or agency-based program
(1) if the agency does not meet the requirements in the department's Personal Care Assistance Agency Certification Application Packet, adopted by reference in 7 AAC 43.793;
(2) for grounds and under procedures set out in 7 AAC 43.950 - 7 AAC 43.985; or
(3) if the agency is no longer qualified for certification under this chapter.
(b) Providers that are disenrolled or decertified by the department under (a)(2) or (3) of this section may appeal that decision under 7 AAC 43.980.
