(a) Based on considerations of cost and clinical effectiveness, the division may require a prescriber of a specific drug to obtain prior authorization from the division for the dispensing of the drug before payment will be made. In emergency situations, the division may approve the dispensing of, and payment for, up to a 120-hour supply of a prescribed drug requiring prior authorization. The division will respond to a request for prior authorization within 24 hours after the time the request is received.
(b) As necessary to prevent waste or to address fraud and abuse, the division may place limitations on the minimum or maximum quantities allowed of a specific prescribed drug or a therapeutic drug class, or on the number of refills of a specific prescribed drug or a therapeutic drug class.
