(a) Payment may be made for reserving a bed during a temporary absence of a recipient from the facility for leave of absence only. The planned absence and times of departure and return of the recipient shall be recorded in the facility's records.
(b) The division will not pay for more than 12 consecutive days of leave of absence without written prior authorization from the division. The division will not pay for total absences in excess of 12 days within a 12 month period per recipient without written prior authorization from the division. The facility must submit a request in writing to the division for any leave of absence in excess of 12 days and receive written approval from the division before the commencement of the leave. Authorized leaves include visits with relatives and friends of not more than 12 days' duration, and leave to participate in therapeutic or rehabilitative programs, which include:
(1) trial visits to alternative care settings to determine if permanent placement is feasible;
(2) gradually increasing lengths of visits to prepare recipients for return to their home or community; and
(3) extended absence to participate in workshop evaluation for rehabilitative programs.
(c) The purpose and plan of all therapeutic or rehabilitative leave must be documented in the plan of care at the facility for the recipient.
(d) When a recipient leaves a facility for hospitalization, payment will not be made to reserve the recipient's bed at the facility.
