7 AAC 34.090. Updating information

(a) In accordance with AS 17.37.010 (g), a patient to whom a registry identification card has been issued must notify the department of a change in the patient's name, address, physician, or primary care-giver within 10 days of the change.

(b) In accordance with AS 17.37.010 (h), a patient to whom a registry identification card has been issued shall return the card to the department within 24 hours of being diagnosed by the patient's physician that the patient no longer has a qualifying debilitating medical condition.
