(a) The protocol shall contain detailed information about the persons, association, partnership, corporation or other organization administering the program. To establish necessary continuity of operation of a program, evidence of stability and financial responsibility must be provided. Articles of incorporation must be included with other information.
(b) The names of persons, such as the director and the pharmacist who have been approved by the federal government to operate a program or to obtain methadone shall be submitted to the department.
(c) A budget shall be submitted with the protocol each year for annual approval if state or federal funds are being sought.
(d) If a patient fee is charged, the fee schedule shall be set forth in the protocol.
