(a) A facility shall have systematic procedures for the review of the quality of care and the use of services and facilities.
(b) The governing board of a facility, or its program director, shall maintain written descriptions of current quality-assurance procedures and shall review and revise the description annually.
(c) If a facility employs a mid-level practitioner, the mid-level practitioner's collaborating physician, if any, must perform on-site quality reviews at least quarterly.
