(a) The governing board of a facility shall provide the division with a table of organization that
(1) identifies all operating units of the program;
(2) defines the roles and responsibilities of all facility staff; and
(3) delineates the interrelationship of the governing board, the program director and all facility administrative and service staff.
(b) The governing board of a facility shall provide the division with the name, address, and telephone number of the secretary of the governing board and the program director, and the name and address of each member of its governing and advisory boards.
(c) The governing board shall annually review and update the information described in (a) and (b) of this section, and shall update the information at any time upon the request of the department.
