A facility which operates primarily for the purpose of providing for the diagnosis, treatment or habilitation of persons who are mentally retarded or have related conditions and which provides, in a protected residential setting, individualized ongoing evaluation and planning, supervision 24 hours a day, and coordination of health and habilitation services to help each individual reach maximum functioning capability is an intermediate care facility for the mentally retarded.
