The clerk of each superior court shall make such reports, and complete and forward such records and certificates as are required by the Vital Statistics Act, related laws, regulations, and instructions. Among others, such reportable court actions shall include decrees of adoption, or vacation or amendment thereof; decrees of divorce or annulment of marriage; orders establishing a delayed certificate of birth; adjudications of paternity; order of change of name; and presumptive death certificates pursuant to AS 09.55.050 , or vacation or amendment thereof. Such reports, certificates, and other requirements shall contain such necessary information; they shall be submitted on forms furnished by the bureau or in such other form as shall be designated; and shall be forwarded monthly or oftener, and by such dates, as directed; all in accordance with instructions issued by the State Registrar after consultation with the Alaska Court System.
