The local registrar shall use, and permit the use of, only those forms approved and supplied by the bureau for any certificates and reports required by the Vital Statistics Act. He shall keep on hand a supply of necessary forms in order to make them readily available to those who are required to complete and file vital statistics records. The local registrar is responsible for any waste or misuse of such forms by those to whom he distributes them, and he is accountable to the bureau for such forms and supplies issued to him.
