In accordance with the Vital Statistics Act, the State Registrar shall prescribe, and the bureau shall reproduce and supply, all forms required by the act and by regulations and instructions based thereon, as well as those required for any other duties assigned to the bureau. No other forms shall be used except with the expressed approval of the State Registrar. When a new form is issued, the State Registrar may recall all old unused forms, or otherwise order their disposal. All forms and supplies issued remain the property of the bureau, which may require an accounting thereof if necessary; and they shall be surrendered to the State Registrar upon his demand. Each form shall be used only for the purpose designated, and not for any other purpose.
