(a) Form 21-405 and Form 21-406 are adopted by the department for use in application for the senior citizen and disabled veteran renter property tax equivalency payment, respectively. Form 21-405a and Form 21-406a "rent certificate" are adopted by the department as supplements to Form 21-405 and Form 21-406, respectively.
(b) Application forms will be supplied upon request by the office of the state assessor.
