A written notice informing a potentially eligible applicant or insured of the rate reduction available under AS 21.89.025 must include
(1) a statement that, to be eligible for a discount, the principal operator of the motor vehicle covered by the insurance policy must
(A) be 55 years of age or older;
(B) request the insurer to provide the discount at renewal;
(C) have had no chargeable accidents as set by the established underwriting guidelines in use by the insurer;
(D) have had no moving motor vehicle citations within three years preceding the request for the discount;
(E) have successfully completed a motor vehicle accident prevention course approved by the Department of Public Safety within three years preceding the request for the discount; a course taken and completed as a result of an order or sentence imposed by a court may not satisfy this requirement; and
(F) provide to the insurer documentation issued by the sponsor of the motor vehicle accident prevention course that states the date that the course was taken and successfully completed;
(2) the circumstances under which the insurer's rating rules permit the insurer to cancel the discount if the principal operator of the motor vehicle is
(A) involved in an accident caused by the operator; or
(B) convicted of a moving vehicle violation;
(3) a statement that the discount may not extend beyond three years after the last day of the operator's most recently successfully completed motor vehicle accident prevention course; and
(4) the insurer's premium discount calculation.
