(a) At the time a licensee applies for license renewal, a licensee shall submit a notarized affidavit stating that the license has complied with the continuing education requirements of 3 AAC 23.100 - 3 AAC 23.170, on a form supplied by the director.
(b) If an affidavit of compliance is not received by the director by the license renewal date, the license lapses.
(c) The licensee is responsible for maintaining adequate and detailed records of all continuing education credit hours claimed on the affidavit and shall make the records available to the director upon request. The licensee shall retain these records for two years from the license renewal date for which the continuing education credits are submitted and for four years if the credits are to be applied to the next license renewal period.
