(a) A person wishing to do business as a small loan company under AS 06.20 must submit a written request to the division for an application for licensure form and a bond form and indicate in the written request the location at which the applicant proposes to do business.
(b) Only fully completed applications on the department's application for licensure form and bonds on the department's bond form are to be submitted to the division for department approval. Whenever space on either form is inadequate, for a full and complete answer, the answer must be continued on a separate sheet attached to the form.
