AS 28.35.135. Unlawful to Knowingly Make False Statement, Application, or Certification.
- (a) A person may not knowingly make a false affidavit, statement, or representation, or affirm falsely with respect to a
matter or fact required to be set out under this title, nor may the person use a name other than the person's true
name. A person convicted of violating this section is guilty of unsworn falsification in the second degree and is punishable as prescribed
by law.
- (b) A person who has a certification, registration, title, license, or other form issued under this title, or who has
applied for a certification, registration, license, or other form, and who changes the person's name or moves from the
address shown on the department's records or forms, shall notify the department of the change in name or
address within 30 days
- (1) on a form or in a format specified by the department; and
- (2) in a manner prescribed in regulations adopted by the department.
